Home Services Older adults

Older adults, with time to talk.

Medicines that have accumulated, memory that is not what it was, balance, hearing, and how home is actually going. The appointment that covers all of it is funded by Medicare every year from 75.

Open Monday to Friday, 8:30am – 5:00pm. No referral needed to see a GP.

The list gets longer, and so should the appointment.

By seventy-five most people are managing several conditions, a list of medicines from more than one prescriber, and a set of small changes that are easy to dismiss individually — a bit unsteady, a bit forgetful, not hearing as well.

Taken separately none of them look urgent. Taken together they are usually the thing that determines whether someone stays independent. That is what the annual health assessment is for.

Bring these to the appointment

  • Every medicine, in the box — including anything from the supermarket or chemist.
  • Any specialists you see and roughly when you last saw them.
  • Your glasses and hearing aids, if you use them.
  • Anyone who helps you at home, if they are willing to come.
  • Any falls in the last year, even the ones that did not cause an injury.

What it covers

What we look at.

The annual assessment covers all of these. Individual appointments can cover any of them.

The big one

Medication review

Medication review

Medicines accumulate. Some no longer suit, some interact, and some were started for a reason that has passed. A proper review often means stopping things as well as starting them.

Asked directly

Memory

Memory

Concerns about memory are worth investigating rather than accepting. Some causes are reversible — thyroid, vitamin levels, medicines, depression, hearing loss — and knowing either way helps with planning.

Preventable

Falls and balance

Falls and balance

A fall is rarely just bad luck. Balance, strength, vision, medicines, blood pressure on standing and the layout of the house all contribute, and most are modifiable.

Quietly disabling

Hearing and vision

Hearing and vision

Both drift slowly enough to be normalised. Both affect memory, mood and safety, and both have straightforward interventions.

Where it applies

Bone health

Bone health

Medicare covers a bone density scan from 70, and earlier where there is a specific reason. Worth knowing before a fracture rather than after.

Practical

Home and support

Home and support

How things are actually going at home, whether help would be useful, and what the pathway is for home care assessment. We can write what is needed.

How it works here

Practical matters.

What does the 75-plus assessment cover?

Medicines, memory, mobility and falls risk, hearing, vision, continence, mood, nutrition and how you are managing at home. It is available every twelve months and it is bulk billed here where you meet the criteria.

Can a family member come in with me?

Yes, and it often helps — a second set of ears is genuinely useful. You can also ask them to step out for part of it.

Can you help with aged care paperwork?

We can provide the medical information that assessments and applications require. Mention it when you book so enough time is allowed, as some forms take a while.

What if I cannot get to the clinic?

Ring us and talk to reception about the options. Telehealth is available for eligible patients who have been seen here in the past twelve months, though some things genuinely need an in-person look.

Questions

Older adults questions.

Is the 75+ health assessment free?

It is bulk billed for patients with a valid Medicare card where you meet the criteria. It is available once every twelve months.

How often should medicines be reviewed?

At least yearly for most older adults, and sooner after any hospital admission or new prescription. Bring the boxes rather than a list where you can.

I have had a fall. Should I mention it?

Yes, even if you were not hurt. Falls usually have several contributing causes and most of them can be addressed. It is one of the most useful things you can raise.

Can I get a bone density scan?

Medicare covers a bone density scan from age 70, and earlier where there is a specific indication such as a fracture after a minor fall or long-term steroid use. Your GP will advise whether it applies to you.

Can a carer or family member attend my appointment?

Yes. Bring whoever helps you day to day if that would be useful, and you can ask them to step out for part of the consultation.

This page is general information for people in the Gosford and Central Coast area. It is not advice about your situation and it does not replace a consultation. If you are worried about a symptom, book an appointment or call us on (02) 4311 3800. In an emergency, call 000.

Book the long appointment.

Ask reception for a health assessment and mention your age, and bring your medicines in the box. All Medicare-eligible consultations are bulk billed for patients with a valid Medicare card.
