How often should a care plan be reviewed?

It depends on the condition and how stable it is. The plan itself sets the interval, and having that appointment booked is much of the point.

Is a care plan review bulk billed?

Yes, where you meet the Medicare criteria and hold a valid Medicare card.

What should I bring?

Every medicine you take, ideally in the box, plus any results or letters from other providers since your last review.

Do I need a review if nothing has changed?

Usually yes. Plenty of important changes produce no symptoms at all, and the review is where they get found.

Can the review be done by telehealth?

Sometimes, depending on what needs checking. Ring reception and ask.
